From Ignis Effigia in Perdition Rock, he's a Phantom. Run east from Ember Light camp, and he'll spawn about 1" from town. Just make sure you don't aggro the Hydras on the way. You may have to reload the map a few times to get him to spawn.

Actually, he's pretty much always somewhere on the map, usually with a flesh golem boss.
